How to use expat health insurance for German Visas

## Time limitation of expat health insurance


Our "newcomer" plan for expat health insurance can be used for up to 5 years starting from your arrival date in Germany. If you’ve already been in Germany for 2 years and just now apply for expat health insurance, that means you’ll only have 3 years left before your expat health insurance expires. 

The time limit applies to everyone regardless of visa.

## Working holiday visa or Youth mobility visa („Working Holiday“\-Programm)


Because this visa is only available for one year, as long as you haven’t been in Germany for more than 4 years, you can use expat health insurance unless you have [pre\-existing conditions](#pre-existing-conditions1).

## Freelance visas (Visum zur Selbstständigkeit)


If you’re applying for your first\-time freelance visa, then we recommend you get public health insurance unless you’re *not* coming from another EU country, then you would need to consider private health insurance or expat health insurance. If you have [pre\-existing conditions](#pre-existing-conditions1), [expat insurance](https://feather-insurance.com/health-insurance/expat/short-term) probably won’t be accepted for your visa application.

## German tourist visa (Schengen Visum)


If you’re planning to come to Germany for only a few weeks, getting simple [travel insurance](https://feather-insurance.com/travel-insurance) will be just fine while you’re here. Still, if you plan to stay for more than one month, expat health insurance is perfect for you since it covers you throughout the entire EU and can be canceled on a monthly basis. 


For some passport holders, this travel visa is granted on arrival. For others, you’ll need to contact the German embassy in your country to apply. With expat insurance, you can apply online from anywhere around the world, so there is no need to worry about any paperwork.


## Visitation visa (Besucher Visum)


If you have a formal invitation to Germany from a friend, family member, event, or company, you’re still going to need health insurance that covers at least €30,000 in costs, which expat fulfills – Depending on the German city you’re applying in. To find out the specific requirements for your visa, go to the specific city’s visa website and look for the visitation visa. ## Medical visa (Aufenthaltserlaubnis zur medizinischen Behandlung)


For people seeking medical treatment in Germany, the requirements for a visa are vastly different from city to city. The general rule is that you’ll need health insurance to cover you while getting medical treatment, but since expat health insurance doesn’t cover [pre\-existing conditions](#pre-existing-conditions1) (a requirement for this visa), you’ll need to prove that you’re able to cover the costs of the surgery through another insurance or through bank statements. 


You will need at least travel insurance during this time, though (expat health insurance will not work in this case).


## Student visas (Visum zum Studieren)


If you’re 29 years old or younger, we highly recommend signing up for [public health insurance](https://feather-insurance.com/health-insurance/public). 


On our website, you can see four different public health insurance providers, which you can choose from. All of them are nearly identical in terms of coverage with small perks like travel vaccinations. 


This is only possible if your university is accepted by the state. If it is a private institution and not approved you will have to take out [expat insurance](https://feather-insurance.com/health-insurance/expat/short-term). But don’t worry, we will check that for you in the sign\-up process.


If you’re 30 years old or older, then public health insurance won’t accept your [student insurance](https://feather-insurance.com/health-insurance/public/students) status unless you’re working for the university as a Ph.D. student. Your university will have specifically told you that you would be covered under their insurance as a researcher or guest academic. ## Visas for employment (Blaue Karte EU)


Expat insurance is not an ideal solution and doesn’t always work for your visa appointment unless you have a very specific situation. It’s best first to see if you would be covered by public or private health insurance before applying for expat – which you can check on our [health tool](https://app.feather-insurance.com/recommendation-tool/health). 


Someone might need expat insurance because they’re coming to Germany on a temporary employment visa and need to apply before starting work. Expat insurance can be canceled on a monthly basis, so you can use it until your employer enrolls you into a public or private health insurance plan. 

## Chancenkarte, opportunity card, or job searching visa


If you [qualify for a Chancenkarte](https://feather-insurance.com/blog/chancenkarte-how-to-apply/), you almost always have less than 2 years to find a full\-time position. This means that [expat health insurance](https://feather-insurance.com/health-insurance/expat/short-term) is perfect for this period of time when you don’t qualify for public or private insurance.

## Visa renewals

If you’re getting a visa renewal, you should either:
- Switch to public health insurance 
- Switch to private health insurance 
- Switch to long-term expat health insurance 

## Pre\-existing conditions


In the information document on the application (this is available on the website of the Foreigner’s Office in the city you’re applying in), they will almost always deny expat health insurance if you’d pay more than €300 out of pocket per year, which is the case if you have pre\-existing conditions. Since expat insurance doesn’t cover pre\-existing conditions, this ends up being a problem. Pre\-existing conditions include chronic illnesses like lung disease, kidney failure, or diabetes that an expat plan won’t cover.

## How much does physiotherapy cost in Germany?


Physiotherapy prices in Germany are generally affordable, averaging €30 per session, but the exact costs vary depending on the type and length of treatment and the physiotherapy center’s location.


Here’s a general idea of what you can expect to pay:

## Does German health insurance cover physiotherapy?


**Yes, if your doctor prescribes it.** 


However, the amount they cover (and how easy it is to access care) depends on your insurance type.


### With public health insurance (GKV)


Public insurers like TK or AOK typically cover **up to 90%** of the cost of prescribed physiotherapy. 


Here’s what that usually means:


- **You need a prescription** from a general practitioner or specialist
- **You pay a co\-payment**: 10% of the treatment cost plus €10 per prescription.
- Coverage is limited to medically necessary treatment.
- Maximum of 6–10 sessions per prescription, extendable if needed


For example, if a session costs €50 and you have 6 sessions, the total is €300\. Insurance pays €270, and you pay €30 \+ €10, meaning a total of **€40 out\-of\-pocket**.


### With private health insurance (PKV)


Private health insurance in Germany generally offers **comprehensive coverage** for physiotherapy.


To qualify for reimbursement, three conditions typically need to be met:


- A **licensed medical doctor** has issued a prescription (indicating the treatment is necessary to treat or alleviate a health condition)
- The treatment is delivered by a **registered healthcare provider**, such as a licensed physiotherapist, not a medical doctor.
- **Therapy is billed according to the GOÄ** (Germany’s [official medical fee schedule](https://www.dguv.de/medien/inhalt/reha_leistung/verguetung/physio-a-positionen-01.04.2023-mit-uv-goae-nrn.-fuer-zabas.pdf)). If you’re ever in doubt about this, ask your therapist for a cost plan and submit it to us for pre\-approval.


Coverage specifics can vary between companies and your specific plan, but here’s a breakdown for Feather’s private health insurance, which should give you a general idea:

- There’s a €30 co\-payment per prescription.


As with short\-term plans, the condition must have started after your coverage began, and treatments like chiropractic or osteopathy are excluded.


## How to get physiotherapy in Germany


Here’s how it works:


### 1\. Get a referral


Before booking sessions, you’ll need a **referral (prescription)** from a licensed medical doctor, like a GP or specialist. The referral must state that the treatment is **medically necessary**, and typically includes:


- The diagnosis or reason for treatment
- Number of sessions prescribed
- Type of therapy recommended (e.g, physiotherapy, manual therapy, etc.)


Please note that referrals are usually valid for **28 calendar days** from the issue date, so don’t wait too long to book your first session.


### 2\. Find a physiotherapist


Once you have a referral, the next step is booking your sessions.


However, finding the right physiotherapist in Germany can take some time, especially if you’re new to the country or don’t speak fluent German.

- **Accepts your insurance plan**, especially if you’re on public insurance.
- **Charges within GOÄ limits** (Germany’s medical fee scale) to ensure bills will be fully reimbursed.

### 3\. Get a cost plan approved


If you have **private or expat health insurance**, you must **submit a cost plan** before starting physiotherapy. 


With it, your insurer will **confirm your** **coverage in advance** and can save you from unexpected out\-of\-pocket costs.


It’s a document prepared by your therapist that outlines:

Aside from these, most necessary treatments are free at the point of care. These are typically not covered and must be paid in full out of pocket.

If you’re applying for public health insurance through Feather, the process usually moves along quickly. 


Here’s a basic idea of how long each step takes:


### Application submission


After you submit your application, you’ll receive a confirmation email immediately. 


This is just to let you know your application was received—it is not an official coverage approval.


### Getting your confirmation of coverage


Official enrollment usually takes about **two weeks**. 


It might be shorter if you have a full\-time job and it is your first time getting insurance in Germany, but it could be longer if additional documents are required. 


You’ll receive an email that includes your policy number and an official membership confirmation. You can use this membership confirmation for all administrative tasks and medical treatments.


### Getting your insurance card


After you’re officially enrolled, the insurance company will ask for a passport photo to create your insurance card. 


Getting your physical card can take up to **eight weeks**, but no worries—you’re covered before it arrives. 


In fact, you’re covered as soon as you receive the confirmation of coverage.


## **It’s been a few days and I haven’t been approved. Why?**


Sometimes, the process takes more time. The most common reasons include:


- **Additional documents are required:** If you’ve had health insurance from another EU country, the insurer may need extra paperwork to confirm your previous coverage.
- **Employer timing:** Some employers might register you with the health insurance at the end of your first month, which can slow things down a bit. However, when they do, they’ll backdate the start of your insurance, meaning that you are covered regardless.


This is generally because German insurers ask for additional documentation (like a letter from a previous EU\-country insurer or GP) to prove previous coverage. 


## What can I do to speed up the process?


If you have been waiting and are tired of doing so, here are a couple of things you could try:


- **Talk to your employer:** Sometimes, employers forget a step while registering you, especially if they outsource services like HR or accounting.
