## Health insurance and the BSN explained

When you move to the Netherlands, one of the first questions is how health insurance works and what role the **BSN (Burgerservicenummer)** plays.

The BSN is your Dutch **citizen service number**, similar to a social security number in other countries. You will need it for everyday things like paying taxes, opening a bank account, and arranging healthcare.

Because the BSN connects you to government systems, it is also required for taking out **basic Dutch health insurance (basisverzekering)**, the mandatory plan for most residents. It is also required to sign up with the Digital ID. Knowing why this number matters, and what to do if you do not have one yet, will help you avoid being uninsured.

## Do you need a BSN to get Dutch health insurance?

In most cases, yes. To apply for **Dutch basic health insurance**, you must have a BSN and be registered with your municipality (you do this right after arrival when you have your Dutch address). The BSN links your policy to government records so that contributions and benefits are tracked correctly.

The Dutch Health Insurance Act (**Zorgverzekeringswet**) requires you to arrange insurance within four months of moving or starting work. Your policy should start from the date your obligation begins, not from when you sign the contract.

Without a BSN, you cannot take out the mandatory basic plan. This is why many newcomers look for alternatives until they are fully registered.

## Who must take out Dutch health insurance?

Most people who live or work in the Netherlands must be insured. If you are registered as a resident or if you earn income here, you are usually obliged to arrange **Dutch health insurance with a BSN**.

- **EU citizens** may use an **EHIC (European Health Insurance Card)** or **S1 form** at first, but must switch if they stay long term.

## What happens if you do not get a BSN on time?

The **CAK (Centraal Administratie Kantoor)** checks whether people have health insurance. If you are required to be insured but do not act, the CAK will send a letter.

- After three months, you can be fined around four hundred euros.
- If you still do not arrange insurance, the fine may be repeated.
- Eventually, the CAK can register you with a default insurer for twelve months and deduct premiums directly from your income.

Delaying BSN registration or insurance can therefore become very costly.

## Who cannot get Dutch health insurance?

Not everyone is simply delayed in the process of being able to sign up for the Dutch Basic. Some groups are **not eligible** for Dutch basic health insurance at all. Here are some examples

- Many international students are not seen as residents under the Dutch Health Insurance Act. Even if they have a BSN, they cannot take out the basic plan. They need private or student health insurance instead.
- Individuals on a orientation year Visa
- Individuals who spend more than 185 days a year outside of the Netherlands
- Others, such as job seekers or short term workers, may need temporary cover until they are fully registered.

A correspondence address does not count as a permanent home, but it can help you keep your BSN active and avoid gaps in your health insurance.

### **If you’re employed**

Your health contributions are handled automatically through your salary.

Your employer pays a percentage straight to the tax authority on your behalf, and you only pay the monthly premium for your basic plan directly to your insurer.

So, in short:

- Contributions are managed through payroll
- You don’t need to take extra action
- Your employer covers the income-based part

### **If you’re self-employed or freelance**

You handle everything yourself. That means you pay:

- A monthly premium to your chosen insurer

The population is getting older. The average age was **42.8 years in 2024**, up from **33.2 years in 1975**.

**16.8%** of residents (around **3 million people**) were born abroad. The other **15 million** were born in the Netherlands, and **2.1 million** of them are **children of migrants**. In total, **11.7%** of the population was born in the Netherlands to at least one parent born abroad.

### How many young people are there in the Netherlands?

In 2025, **4,904,057 inhabitants (27.2% of the population)** were under 25. They're not spread evenly across the country, though.

### Life expectancy statistics in the Netherlands

In 2024, average life expectancy in the Netherlands was **81.9 years**, 0.8 years above the OECD average. Men could expect to live to **80.5**, and women to **83.3**.

That's a long way from **1950**, when average life expectancy was **71.5 years** (**70.3 for men** and **72.6 for women**).

## How much does the Netherlands spend on healthcare?

In 2023, the Netherlands spent **9.8% of its GDP on healthcare**, just under the EU average of **10%**. Per person, though, it spent far more: **€4,848**, around **25% above the EU average**.

By 2024, total healthcare spending had climbed to **€113.5 billion**. That's an **€8.5 billion (8.1%) increase** on the year before. Healthcare grew faster than the economy as a whole, pushing spending up to **10% of GDP** and bringing the Netherlands in line with the EU average.

## Where does the Netherlands spend its healthcare budget?

The biggest slice goes to outpatient care, at roughly **33.3%** of healthcare spending.

Long-term care comes next at **29%**, well above the EU average of 18%. Inpatient care takes **18%**, and pharmaceuticals **10%**.

Spending on prevention jumped during the COVID-19 pandemic, from **3.3%** of total health spending in 2019 to **8.7%** in 2021. It fell back to **5.2%** in 2023, but that's still above the EU average of **4.2%**.

The pattern is clear: the Dutch system puts outpatient and long-term care ahead of hospital stays and medicines.

### How much does the Netherlands spend on pharmaceuticals?

Not much, by EU standards. That's true both in total and as a share of healthcare spending.

In 2023, retail pharmaceutical spending came to **€6.7 billion**, or **€309 per person**. That's the lowest in the EU: nearly half the EU average, and less than **15%** of total healthcare spending.

### How are pharmaceuticals funded in the Netherlands?

Mostly through social health insurance, which covered **68%** of retail pharmaceutical spending. That's above the EU average of **62%**, but below France (**83%**), Germany (**83%**) and Belgium (**74%**).

Patients paid **31%** of retail pharmaceutical costs out of pocket in 2023. That's below the EU average of **36%**, but noticeably higher than France and Germany, where out-of-pocket payments made up **12% and 17%** respectively.

## Where does healthcare funding in the Netherlands come from?

Coverage in the Netherlands is close to universal. **99.9%** of the population is covered by statutory health insurance, which pays for most types of care:

* **94%** of inpatient care costs were publicly financed in 2023, above the EU average of **91%**

* **79%** of outpatient care spending was publicly funded, slightly above the EU average of **77%**

* **68%** of pharmaceutical spending was publicly financed, compared to **59%** across the EU

Dental care is the big exception. Only **33%** of dental costs were publicly financed in 2023, slightly below the EU average of **35%**. So if you need dental work in the Netherlands, expect to pay more of it yourself.

## How healthy are people in the Netherlands?

Healthier than the EU average on most counts. **13.2%** of Dutch adults smoke daily, and they drink an average of **7.8 liters of alcohol per person** a year. The EU averages are **18.5%** and **9.8 liters**.

**13.6%** of Dutch adults are obese, just below the EU average of **14.6%**. Education makes a big difference here: people with the lowest education level **are more than twice as likely to smoke and be obese** as the most highly educated.

**6%** of people in the Netherlands rate their health as bad or very bad, a little below the OECD average of **8.0%**.

### How many people in the Netherlands have a chronic health condition?

Around **58% of the population, or 10.5 million people**, had at least one chronic disease in 2022. By 2025, that had risen to **10.9 million people**.

In 2024, **5.6 million people (31% of the Dutch population)** saw their GP at least once about a chronic condition.

Age is the biggest factor. **96% of Dutch residents over 75** have a chronic condition, compared to **45% of those under 40**.

### Cause of death statistics in the Netherlands

In 2023, **168,764 deaths** were recorded in the Netherlands. Cancer (malignant neoplasms) was the single largest cause, responsible for **27.1%** of deaths. Cardiovascular diseases came second at **22.4%**.

Alzheimer's and other dementias were the third leading cause, at **9.9%** of deaths.

Falls accounted for **3.7%** of deaths and suicide for **1.2%**. Only **0.5%** of deaths were due to transport accidents.

## How many doctors and nurses are there in the Netherlands?

In 2023, the Netherlands had **3.9 practicing doctors per 1,000 people**, up from **3.4 per 1,000** in 2014. Even so, it's still below the EU average of **4.3 doctors per 1,000 people**.

One thing that stands out is the focus on primary care. Around **one in four doctors** in the Netherlands works as a general practitioner (GP).

Nurses are a different story. Numbers have grown steadily, from **10.3 nurses per 1,000 people** in 2014 to **11.1 per 1,000** in 2023. That's well above the EU average of **8.4 nurses per 1,000 people**.

Overall, the Netherlands has more nurses per person than France, but fewer doctors and nurses than Luxembourg, Germany, Iceland and Norway.

### What percentage of the population has unmet medical needs?

Very few people in the Netherlands go without the care they need. But the picture changes when you look at income and the type of care.

In 2024, **1.6%** of the population reported an unmet medical need because of cost, distance or waiting times. That's less than half the EU average of **3.5%**.

Among people below or at risk of poverty, **4.3%** reported unmet medical needs. Unmet dental care needs affected **2.6%** of the population, rising to **6%** among lower-income households.

The biggest gap is in mental healthcare. About **9%** of people in the Netherlands reported unmet mental health needs, above the EU average of **7%**, despite strong coverage overall.

### How many hospital beds are there in the Netherlands?

The Netherlands has one of the lowest levels of hospital capacity in the EU. That's by design: the system has long focused on outpatient and primary care instead.

Hospital beds fell from **3.2 per 1,000 people** in 2017 to **2.3 per 1,000** in 2023. That's less than half the EU average of **5.1 beds per 1,000 people**.

In 2023, the Netherlands had **41,310 hospital beds**, down from **43,355** in 2022. The longer trend is even clearer: in 2015, the country had **59,117 hospital beds**.

Fewer beds fits the Dutch model, which favors outpatient care, shorter hospital stays and treatment outside traditional hospital settings.

Hospitals are also used less. The country had around **8,000 hospital discharges per 100,000 people** in 2023, half the EU average. Bed occupancy stood at **60%**, below the EU average of **68%**.

### How many hospitals are there in the Netherlands?

In 2022, the Netherlands had **733 hospitals**, up from **691** in 2021. That continues a long-term rise from **198** hospitals in 2005. (Fewer beds, but more places to get care.)

## How many people are employed in the Dutch health and welfare sector?

In 2023, an estimated **1.63 million people** worked in Dutch health and welfare, up from **1.6 million** in 2022. Go back to 2001 and the figure was **1.05 million**.

That means around **580,000** extra people joined the sector between 2001 and 2023.

Not every group is growing, though. The number of self-employed healthcare workers has dropped over the last few quarters.

In the five quarters since Q3 2024, the Netherlands lost around **4,000 self-employed healthcare workers**, a **13%** decline. The steepest fall was in elderly care, where the number of self-employed workers dropped by **63%**.

In 2023, the average employee in the Netherlands took around **14 days of sick leave**.

That's a slight increase on the COVID period, when employees took roughly **12 sick days** a year. The rise matches growing concern about stress-related illness and mental health at work.

Work-related stress is now a major driver of sick leave in the Netherlands. An estimated **25%** of all sick days taken by Dutch workers are put down to stress, the highest level since the middle of the COVID-19 pandemic.

## Who needs a visa? 

If you’re planning a trip or a move to the Netherlands, the first thing to figure out is whether you need a visa at all. 

The rules vary depending on your nationality and the duration of your stay.

If you’re a **citizen of the EU, EEA, and Switzerland**, you can enter the Netherlands freely without a visa or residence permit, even for long stays.

For most other countries, you’ll need to apply for a visa.

The Netherlands has two types of visa:

* **Short Stay Schengen visa**: These are for anyone travelling to the Netherlands (or anywhere else in the Schengen are, for 90 days or less (with a 180-day period).   
* **National long-stay visa**: You’ll need one of these if you’re travelling long-term for work, study, or family reunification. They often require sponsorship from an employer, school, or family member.

For short stays, some countries, such as the US, Canada, Australia, and Japan, have agreements that allow their citizens to visit the Schengen Area visa-free for up to 90 days within a 180-day period.

## Required documents for a Dutch visa

No matter which visa you’re applying for, there are certain documents that you’ll likely be required to provide. 

These include:

* **A completed visa application form**: These can be downloaded from your local Dutch embassy or consulate website. Some allow online submission, while others still require paper copies.   
* **A valid passport**: Must be issued within the past 10 years, and be valid 3 months beyond your planned departure.   
* **Biometric photos**: 35x45cm, against a plain light background, with a neutral facial expression.  
* **Proof of accommodation**: Hotel bookings, rental contracts, or a notarized invitation from a host in the Netherlands.   
* **Travel itinerary**: You don’t usually need to buy flight tickets before applying. A reservation showing your planned entry and exit is often enough. This protects you in case your visa is refused.   
* [**Visa-compliant health insurance**](https://feather-insurance.com/en-se/health-insurance/expat): Must cover at least €30,000 in medical expenses, including emergency expatiation, across the entire Schengen area. Technically, this is optional, as you'll need to sign up for the [Dutch healthcare system](https://feather-insurance.com/en-nl/blog/health-insurance-netherlands-guide) as soon as you arrive, but we've seen a higher visa application success rate for our clients in the Netherlands if they have this. 
* **Proof of financial means**: Bank statements, pay slips, or a formal sponsorship declaration. You’ll usually need to prove that you can support yourself at €55 per day.

## Additional documents

Beyond the standard checklist, you might be asked for additional documentation depending on the reason you’re coming to the Netherlands. 

The IND and embassies use these documents to confirm that your plans are genuine and that you meet the conditions for your visa type. 

Here are some instances where you may need to provide additional documents:

* **Visiting family/friends**: An invitation letter will be required. Your host may also need to submit a sponsor declaration (garantstelling) showing that they’ll cover your expenses if needed  
* **Joining family:** You’ll need to provide legal proof of family ties, like marriage or birth certificates. Most of these must be legalized or carry an apostille stamp, depending on your country.  
* **Employed**: Employees must usually provide an employer’s approval letter confirming their trip dates and job security.  
* **Self-Employed**: Self-employed applicants may need to provide tax filings, registration documents, or proof of financial stability. These help prove you’ll return home and can support yourself.  
* **Students**: You’ll need to show proof of admission from a Dutch school or research institute, along with financial information showing how you’ll fund your stay.  
* **Au pairs**: Au pair arrangements must go through an IND-recognized agency. Contracts outside of this system aren’t accepted.  
* **Retirees**: You’ll need to show proof of long-term financial security. This could be pension statements, proof of savings, or other evidence that you can sustainably support yourself.

## Additional documents for minors

When children apply for a visa to the Netherlands, embassies apply extra checks to protect them.

Authorities want to make sure both parents or legal guardians agree to the trip and that the child isn’t at risk of abduction.

Here’s what’s usually required:

* A **signed parental consent form** confirming the child has permission to travel.
* A **copy of the parents’ or guardians’ passports** or ID cards to verify signatures.
* The **child’s birth certificate** establishes the legal relationship.

In **dual custody situations**, both parents must give consent. If one parent cannot be present, their consent often needs to be notarized. 

If **only one parent has custody**, official proof (a court decision or custody certificate) must be submitted. 

Some countries may also require the consent documents to be legalized or carry an apostille. 

Because rules can differ, it’s always best to check the Dutch embassy’s checklist for minors in your country.

## Special requirements for long-stay (MVV) visa

If you’re planning to stay in the Netherlands for more than 90 days, you’ll likely need a **long-stay visa (MVV)**. This process includes additional steps to ensure you’re ready to settle in the country.

One of the biggest is the **civic integration exam abroad**. 

This covers basic Dutch language skills and knowledge of Dutch society, and many applicants must pass this test at the Dutch embassy in their home country before applying. 

The goal is to show you can adapt and understand everyday life in the Netherlands. Some groups, like young children or certain students, may be exempt.

Another key requirement is having a **recognized IND sponsor**. 

This sponsor can be an employer, a Dutch university, or a family member already living in the Netherlands. Only sponsors officially registered with the IND are accepted. 

The process starts with your sponsor applying to the IND. Once the IND approves, they issue an **approval letter (the MVV decision)**. You’ll need this letter in hand when you book your appointment.

## Legalization and translation of documents

When you submit foreign documents for a Dutch visa, they typically need to be **legally recognized** by the embassy or IND before they will be accepted. 

Whether or not your documents need legalisation depends on whether or not your country has signed the Hague Convention:

* **Hague Convention countries:** An **apostille stamp** from the issuing authority is enough to make the document valid in the Netherlands.
* **Non-Hague countries:** Documents must go through **full legalization**. This means approval from local authorities first, followed by the Dutch embassy or consulate in that country.

Additionally, your documents must be **translated by a sworn translator** if they aren’t in Dutch, English, French, or German. 

The exact process depends on where the documents were issued, so as always, it’s worth checking details with your local Dutch consulate or embassy. 

## Biometric data

All applicants, no matter which visa you’re applying for, must provide fingerprints and a digital photo. 

These are usually taken on-site during your appointment. 

Fingerprints are stored for roughly 5 years in the Schengen Visa Information System (VIS), so if you reapply within that window, you may not need to repeat them.

Some applicants, such as certain government officials or children under 6 years old, may be exempt. 

## Visa fees

When applying for a visa, everyone must pay a fee. 

These fees cover the cost of application, and are non-refundable even if your application is rejected. 

Prices differ based on the type of visa:

* **Schengen short-stay visas**: Around €80 for adults, €40 for children aged 6–12, and free for children under 6\.

* **Long-stay MVV visas**: Fees vary widely depending on purpose, usually between **€210–€350**.

**Methods of payment** differ by consulate/embassy. Some only accept cash, whilst others accept card or bank transfers.

Due to these small differences, it’s always worth reaching out to your local Dutch mission beforehand to make sure you aren’t caught off guard. 

## How to apply

Most people apply through the **Dutch embassy or consulate** in their country of residence. 

In many places, the Netherlands works with outsourced providers like VFS Global. These centers handle the intake, but decisions always come from the Dutch authorities.

You can apply as early as **6 months before your trip** (or 9 months for seafarers). The absolute latest is 15 days before departure.

Cutting it close is risky though, and processing times can extend during peak seasons, so applying well in advance is strongly advised.

## Processing times

For **short-stay Schengen visas**, the standard processing time is about 15 calendar days. Some cases are faster, while others may take longer, especially if additional documents are required. 

For **long-stay MVV visas**, expect a more complex process that typically spans several weeks or even months.

Processing times for both Schengen and long-stay MVV do vary, and tend to be longer during peak seasons (July-August and December-January). 

## **Frequently asked questions**

### **Should documents be originals or copies?**

Most Dutch embassies r**equire the originals** of your documents and retain **copies** for their records.

Originals are usually returned during or after the appointment. However, if you submit notarized or legalized originals, the embassy may temporarily retain them for verification purposes. 

Because practices differ slightly from one mission to another, always double-check the **embassy’s specific checklist** before your appointment.

### **What if my passport has less than 6 months' validity?**

For a Schengen visa, the official rule is clear: your passport must be **valid for at least three months beyond your planned departure date and have been** issued within the last 10 years. 

If your passport doesn’t meet these validity rules, your visa application will be automatically rejected.

The best option is to **renew your passport** before applying.

### **Can I apply in a country where I am not a resident?**

Generally, you must apply for a Dutch visa in your **country of legal residence**. This allows the embassy or consulate to properly verify your ties and supporting documents.

There are exceptions. If there’s no Dutch representation in your home country, or if the Netherlands outsources applications to another state, you may be allowed to apply elsewhere. 

In such cases, always confirm with the specific embassy or consulate before booking an appointment.

### **What if I overstay my visa in the Netherlands?**

Overstaying a Dutch visa comes with serious consequences.

Penalties may include **fines, entry bans across the Schengen Area, and future visa refusals**. Every overstay is recorded in EU-wide systems, so it won’t go unnoticed when you apply again.

If your overstay happens because of **exceptional circumstances** (illness, cancelled flights, or other emergencies) it’s important to contact the **IND** as soon as possible. 

The earlier you act, the better your chances of resolving the situation.

### **Do I need a separate visa for traveling to other Schengen countries?**

No, you don’t need a separate visa. A valid Dutch-issued **Schengen visa** lets you travel to all other Schengen countries during its validity period. 

This makes it easy to combine the Netherlands with trips to France, Germany, Spain, or any of the 20+ Schengen states.

## **Choosing the right visa**

Before applying, you’ll need to figure out which visa is right for you. The type you need depends on the length and purpose of your visit.  

If you’re staying in the Netherlands for **under 90 days**, you’ll need to apply for the **short-stay Schengen visa**. These are common for tourism, business trips, or short study programs. 

Short-stay Schengen visas:

* Allow you to travel across all 27 Schengen countries  

* Maintain a **90/180-day rule** (you can stay up to 90 days in any 180-day period)  

* Can be either **single entry** (leave once, it’s over) or **multiple entry** (come and go within the validity period)

If you plan on moving **long-term (more than 90 days)**, you’ll need a **long-stay MVV visa** ([machtiging tot voorlopig verblijf](https://ind.nl/nl/machtiging-tot-voorlopig-verblijf-mvv)). Think of it as the entry ticket for your **residence permit**. 

You apply for the MVV from abroad, typically with the assistance of your sponsor. Once you arrive in the Netherlands, you pick up your residence permit at the IND office.

You typically need an MVV if you’re:

* A **student** enrolled in a Dutch university. Universities usually apply on your behalf once you’re admitted.

* An **employee** with a work contract. Employers typically handle most of the application process, and you provide supporting documents.

* A **family member** joining a Dutch resident or citizen.

One common misconception is that you can enter the Netherlands on a tourist visa and then switch to a work visa within the country. That’s usually not allowed, and you’ll need to apply from outside the country.

## **Step 1: Complete the Dutch visa application form**

The first step is to complete the official visa application form. Where you find this form and how you submit it depend on the type of visa and your location.

For **short-stay Schengen visas**, forms are available on the website of your local Dutch embassy or consulate. Some countries handle applications through [VFS Global](https://www.vfsglobal.com/en/individuals/index.html), which can also provide the forms.

For **Long-stay MVV visas**, the Dutch Immigration and Naturalisation Service (**IND**) provides the forms and guidance directly on their website. 

There are three main ways to submit your application forms :

* **Directly through the IND**: Mostly for residence permits when your sponsor applies on your behalf.

* **At a Dutch embassy or consulate**: You make an appointment, bring your documents, and submit the form in person.

* **Through VFS Global**: In many countries, the Netherlands outsources Schengen visa handling to VFS. You apply via their website and attend an appointment at their office.

**Online applications** are available in some countries, but not all. The safest bet is to check the exact requirements listed on your local embassy’s website.

## **Step 2: Prepare the required documents**

The Dutch authorities tend to be quite strict when it comes to documents. Having everything in order is what makes or breaks your application.

For all visa types, you’ll need:

* A completed and signed visa **application form**.

* **A passport, valid at least 3 months beyond your planned departure** from the Schengen area, with at least 2 blank pages.

* **Two passport photos** (35 x 45 mm)

* **Proof of accommodation**, such as a hotel booking, rental contract, or invitation letter from a host.

* **Travel itinerary**, including flight reservations and any internal travel plans.

* **Financial proof** showing you can support yourself (bank statements, pay slips, or sponsorship letters)
