The agency asks for a fully comprehensive health insurance policy covering you (and any family members) for the full permit year. A refusal happens before your planned move, and therefore before your policy start date. You can cancel the policy in one click from your Feather account, and you'll be fully refunded automatically. Note that the €300 application fee paid to Residency Malta is non-refundable.

Yes. Renewal comes around every year (€300, up to four years total), and Residency Malta asks for proof of health insurance each time. Your Feather certificate works at renewal just as it does after your first approval.

No. Malta's public healthcare is funded through social security contributions from local employment, and Nomad Residence Permit holders work for employers or clients abroad. You'll rely on private health insurance for your whole stay. (UK nationals are a special case: a reciprocal healthcare agreement means Residency Malta exempts them from the insurance requirement.)

No. You can sign up before you arrive in Malta. No Maltese address, bank account, or ID number required. For the permit itself, Residency Malta will ask for proof of a Maltese address within 30 days of your approval in principle, but your insurance can be in place before that.

Pre-existing conditions are not covered. Any illness, symptoms, or treatments that existed before your policy start date won't be reimbursed. This is standard across expat health insurance policies and has no impact on your permit eligibility. No. The permit is strictly for remote work: an employer registered abroad, your own foreign company, or clients based outside Malta. Taking on Maltese clients or local employment would break the permit's conditions. If you want to work locally, you'd need a different permit.
