Whether you’re [self-employed](https://feather-insurance.com/en-ww/blog/international-health-insurance-freelancer), [moving abroad with family](https://feather-insurance.com/en-ww/blog/international-health-insurance-families), [working as a digital nomad](https://feather-insurance.com/en-ww/blog/international-health-insurance-digital-nomad), or [looking to retire](https://feather-insurance.com/en-ww/blog/international-health-insurance-senior) somewhere warm, there are plenty of great reasons to move abroad. But wherever you’re moving, there’s a good chance you’ll need a visa. If so, you’ll probably have to think about health insurance sooner rather than later.

Valid health insurance is a prerequisite for most visa applications. This means you’re going to have to think carefully about what policy you require before making your application.

## United States: health insurance requirements for visas

For expats in the US, there are two main types of visas:

* **J-1:** The main visa used by expats coming to the US for study and work-related activities. It covers activities like employment, internships, or research projects. Generally, applicants must be sponsored by an organization to qualify.   
* **F-1:** This is the main visa used by [international students](https://feather-insurance.com/en-ww/blog/international-health-insurance-student). If you’re pursuing a full-time academic course at a United States college/university, there’s a good chance this is the visa you’ll need.

These visas have different health insurance requirements. Let's look at each one separately.

### United States: J-1 visa health insurance requirements

Most expats in the United States will enter on a J-1 visa. To do so, you will need to demonstrate private health insurance that meets [the following requirements](https://isso.ucsf.edu/j-visa-health-insurance-requirements-and-options): 

* Medical benefits of at least $100,000  
* Maximum deductibles of $500 per accident or illness  
* Maximum co-payments of 25%  
* Minimum medical evacuation coverage of $50,000  
* Minimum emergency repatriation coverage of $25,000  
* Coverage for pre-existing conditions (after a reasonable waiting period)  
* Includes coverage or benefits for activities related to the reason for your visa

Travel insurance policies are rarely accepted here. The chosen insurance provider must also be accredited by one of the four following credit agencies: 

* AM Best (Minimum A-)  
* Insurance Solvency International (Minimum A-i)  
* Standard & Poor’s (Minimum A-)  
* Weiss Ratings (Minimum B+)

### United States: F-1 visa health insurance requirements

Unlike J-1, the F-1 visa does not have health insurance requirements at the federal level. 

Nonetheless, the vast majority of students on this visa will need health insurance anyway. The only difference is it will be managed and enforced by your college/university. 

By default, most colleges/universities require you to have international health insurance. In general, you will be auto-enrolled into a Student Health Insurance Plan (SHIP) that is managed by your education provider. By default, these must be valid for the entire period of study. 

You may opt out in some cases, but only if you can prove equivalent levels of coverage through an alternative health insurance policy.

## Schengen area: health insurance requirements for visas

The situation in the Schengen area can be complex because there are several visa types to consider. At the same time, the rules aren’t exactly the same across all Schengen countries. 

For the purposes of this article, we’ll focus on two main types of visa: 

* **Type C Schengen visa:** Known as the ‘Schengen’ visa or ‘short-stay’ visa. This is the main type of visa used by tourists. It is valid for a maximum of 90 days within a 180-day period. The health insurance requirements for this visa are fairly minimal. You will generally require travel insurance for the Schengen visa.   
* **Type D Schengen visa:** This is the long-stay visa, which applies to anybody staying between 90 days and a year. Unlike the short-stay visa, it is designed for use primarily within a specific country, rather than the entire Schengen area. This visa is most relevant to anybody looking to move to a specific Schengen country as an expat. As such, the health insurance requirements differ notably between different countries. 

As well as sharing these visas, there are several other principles that Schengen countries have in common: 

* **Public and private healthcare:** Most European countries operate a mixed public and private insurance healthcare system. If you’re moving to a European country, you may become eligible for public healthcare while you’re here. If so, you may not need separate, long-term medical insurance. However, public healthcare entitlements don’t become valid immediately. Either way, you’ll still need valid insurance at the point of entry.  
* **Schengen baseline:** A baseline set of health insurance requirements is defined at the EU level. For instance, rules such as ‘€30,000 minimum coverage’ are consistent across the Schengen area.  
* **Type C vs. Type D visa requirements:** Type C requirements do not differ widely across the Schengen area. Type D requirements are generally higher and differ noticeably between countries.

### Schengen visa travel insurance requirements (Type C)

The health insurance requirements for a Type C Schengen visa are defined in [the EU Regulation (EC) No 810/2009, Article 15](https://eur-lex.europa.eu/eli/reg/2009/810/oj/eng). Here are some of the main requirements:

* The policy must offer minimum coverage of €30,000 and be valid across all Schengen countries  
* It should also be valid for the entire period of the person’s intended stay/transit  
* You should be covered for repatriation expenses. This can relate to urgent medical attention, emergency hospital treatment, or death

These rules are less strict than those for Type D visas, because they’re only designed for short stays. Travel insurance is generally valid here – though repatriation coverage must be included.

### Long-stay visa requirements in Germany

Germany has demanding health insurance requirements for long-stay (Type D) visa applicants. Visa applicants must demonstrate valid health insurance at the point of entry. You must also arrange a long-term health insurance solution, which may differ from the policy you used to enter. There are essentially two options for what that long-term arrangement can be:

* **State social security:** This is known as *‘gesetzliche Krankenversicherung’* (GKV). This is the default option for anybody employed by a German organization who earns less than €77,400 per year – including expats. If you’re an expat with a confirmed job offer, your health insurance will only need to cover you until your GKV entitlements become valid.   
* **Private insurance:** Also known as *‘private Krankenversicherung’* (PKV). This is required for anybody who does not qualify for GKV. If you’re an expat, you will likely require private insurance unless you’re employed by a German organization. If you are employed and earn over €77,400 a year, you may choose a PKV insurance package and opt out of the GKV. 

Regardless of the long-term arrangement, you’ll need to demonstrate some form of valid health insurance to enter Germany. If needed, a PKV policy must offer equivalent coverage as Germany's GKV.

Often, expats who aren’t employed by a German organization will not have access to the GKV system. This is particularly relevant to retirees and other non-lucrative expats. If this applies to you, you must maintain full private health insurance indefinitely. 

For self-employed expats in Germany, the situation is a little different. Here, you can opt in to the GKV, but only if you’ve previously been registered under the GKV or another European public healthcare system. In this case, you can choose either GKV or PKV. If you haven’t been registered under a European public healthcare system, you’ll need a long-term PKV policy. 

Given the choice, many self-employed people still prefer PKV as it can result in lower payments in the short term.

### Long-stay visa requirements in France

In France, the long-stay (Type D) visa is known as VLS-TS (Visa de long séjour valant titre de séjour). This enables you to live in France for between four months and a year. Here are some of the [most important health insurance requirements](https://www.frenchentree.com/living-in-france/french-long-stay-visas-which-health-insurance-do-you-need/):

* Travel insurance policies are not accepted  
* The health insurance policy should cover the entire visa length, regardless of how long you want to stay in France  
* Medical expenses, hospitalization, and repatriation must all be covered  
* Minimum coverage of €30,000  
* The policy must not exclude pre-existing conditions  
* There must also be no co-pays or deductibles

The situation in France becomes more complex when we take into account the country’s public healthcare system. This is known as *‘Protection Universelle Maladie’* (PUMa). In France, you’ll become eligible for public healthcare after three months in the country. If you apply for and secure public healthcare, the above requirements are no longer valid. 

However, France’s public healthcare system only covers around 70% of standard medical costs. French residents and citizens therefore take out an additional ‘top-up’ insurance (known as *‘mutuelle’*). This covers the remaining costs in most cases.

If you qualify for public health insurance, you no longer require private insurance. In this case, you may choose to replace your international health insurance with top-up insurance instead. If you’re employed in France, you’ll generally be automatically enrolled in a mutual insurance policy.

### Long-stay visa requirements in Spain

Spain has similar requirements for long-stay visas to France. Here are some of the baseline details: 

* €30,000 minimum coverage  
* The insurer must be authorized to operate in Spain and issue a certificate in Spanish  
* No co-pays or deductibles  
* The insurance policy must cover all risks insured by Spain’s public healthcare system  
* The policy must be valid for at least a year and be maintained for the entire duration of the stay  
* Travel insurance is not accepted

For your visa to be accepted, you must have a valid health insurance policy that fulfills these requirements. 

Like with other European countries, you may qualify for public healthcare shortly after arriving. In Spain, you can do this by contributing to the local social security system – known as *‘Seguridad Social’*. 

If you’re employed by a Spanish organization, you will be enrolled into this by default. For freelancers, retirees, and other non-working expats, the situation is more complex. By default, you do not qualify for public healthcare. Therefore, the private health insurance requirements we listed above will apply. 

However, there is one caveat: *Convenio Especial*. This is a separate scheme that lets you pay into the social security system if you’re not already covered through employment. This is primarily designed for non-lucrative expats and freelancers who don’t pay into the system by default. You may apply for this after one year’s residence in the country. 

## United Kingdom: healthcare requirements for visa applicants

The United Kingdom has a different approach to healthcare and insurance than most European countries. 

Visa applicants do not require private health insurance. Instead, you are required to pay an up-front [Immigration Health Surcharge](https://www.gov.uk/healthcare-immigration-application) (IHS) when applying for the visa. This essentially buys access to the country’s public healthcare system – the [National Health Service](https://www.nhs.uk/) (NHS). 

However, the NHS does not cover all types of treatment. Opticians, dentists, hearing specialists, and prescriptions are not paid for by the NHS – even for UK citizens. Therefore, you may choose to take out private healthcare to cover these additional treatments.

The IHS currently costs a flat rate of [£1,035 a year](https://www.gov.uk/healthcare-immigration-application/how-much-pay#:~:text=%C2%A3776%20per%20year%20for%20students%2C%20their%20dependants%2C,for%20example%20%C2%A33%2C105%20for%20a%203%2Dyear%20visa.). This must be paid up-front, when you apply for your visa. Payment is a prerequisite for the visa being accepted. If the visa you’re applying for will last for multiple years, you must pay all in one go. 

Once you’ve paid this fee, you’ll have access to the full UK healthcare system from day one. You can then interact with it in the same way as British citizens.

But when it comes to visas, healthcare, and other requirements, it’s easy to get overwhelmed by details. After all, there’s a good chance this is your first time living abroad. And the consequences of getting it wrong can be very daunting. The last thing you want is to find yourself with an unexpected bill or a rejected visa.

## What is international student health insurance? 

[International health insurance](https://feather-insurance.com/en-ww/blog/international-health-insurance-guide) is an umbrella term for any private health insurance used by expats. For students, it is often a requirement for your study visa – if you need one.

In reality, ‘international health insurance’ is a loose umbrella term that can mean different things. That’s because there are several factors that influence what health insurance you will require: 

* Which country you’re originally from  
* Where you’re studying  
* What visa (if any) is valid for your stay  
* What existing entitlements/insurance schemes you’re already covered under

If you’re studying in the US, for instance, you will almost certainly require some level of private health insurance. This is usually managed by your university/college. In the EU’s Schengen Area, you will likely need insurance at the point of arrival, but may become entitled to public healthcare during your stay. In the United Kingdom, most visa applicants pay an [Immigration Health Surcharge](https://www.gov.uk/healthcare-immigration-application) as part of the application. 

As you can see, international student health insurance can mean very different things to different people. 

## International health insurance vs travel insurance: What’s the difference?

International health insurance and travel insurance are not the same thing. That’s because the former is designed for expats and the latter is designed for short-stay visas. 

As a student, travel insurance is generally **not valid.** However, it may be accepted for short periods outside of your country of study – i.e. placements, projects, or vacations. 

## Why do students need international health insurance?

There are several different situations where students might need private international medical insurance: 

### Visa requirement 

The most common reason: Medical insurance is often a requirement of your study visa. These rules will differ depending on the country you’re studying in — see our [guide for visa applicants](https://feather-insurance.com/en-ww/blog/international-health-insurance-visa-seekers) for a broader walkthrough of the visa-side coverage rules. For instance, you may need a policy that's valid from before you apply for the visa, before you enter the country, or shortly after. It’s important to check these details beforehand, as getting it wrong can result in your visa being rejected. 

### Short-term coverage

In some countries, you are eligible for public social security coverage shortly after arriving. This is as little as three months in some cases. In this case, you will generally need a short-term international student health insurance plan. This should cover you between your arrival and your social security entitlements becoming valid. 

### Personal choice

You may also choose to take out medical health insurance to increase your coverage. This is particularly relevant if you have existing coverage through the country’s social security system. Otherwise, you might want greater coverage than what’s on offer through your college/university. 

### Short-term placement

If you’re traveling outside your country of study (e.g. vacation, project, or fieldwork), you might need to extend your medical coverage. This depends on the specific terms of your existing insurance policy. Some apply worldwide, and others may be limited to your country of study. 

### Dependents

If you’re bringing dependents with you during your study, you’ll likely need to insure them separately. If you’re entitled to a medical insurance policy through your college/university, they likely won’t be covered. ## What does health insurance for international students include? 

It’s impossible to create a definitive list of what international medical insurance covers and includes. That’s because the details will differ depending on several key factors. These include the type of policy you purchase, your country of origin, and where you’re staying. 

Nonetheless, certain treatments will be common in international student health insurance plans. Others are more common in higher-cost policies. Here are some of the main terms to look out for:

### Basic healthcare coverage

All international health insurance policies will include a baseline level of medical coverage. This could include medical expenses, doctor visits, surgeries, emergency care, and diagnostics. 

### Additional healthcare coverage

Often, additional healthcare requirements are only available in higher tiers of insurance. Depending on the location, this might include dentists, opticians, or prescriptions. 

### Repatriation

This covers the cost of your travel back to your country of origin in the case of death or medical emergency. This often isn’t included in basic packages – so it’s important to double-check this. In the Schengen Area, repatriation coverage is generally a visa requirement, so you may get caught off guard if your insurance doesn’t include it. 

### Preventative care

Your insurance may cover preventative treatments like routine screenings or vaccinations. This is more common in higher insurance tiers. 

### Global coverage

Different policies will apply in different areas. The most basic could just include your country of study. If you’re studying in Europe, you’ll likely have Schengen-wide coverage. Otherwise, worldwide coverage is more common in higher-priced policies. 

## What international health insurance is required for a visa application?

If you’re planning to study in another country, there’s a good chance you’ll require a visa to do so. And often, health insurance for international students is a requirement. 

The specific rules will depend on your country of study. However, it’s helpful to give a brief overview of the visa requirements in some key regions around the world:

### United States

In the United States, students are generally accepted under the [F-1 Student Visa](https://travel.state.gov/content/travel/en/us-visas/study/student-visa.html). You do not require private health insurance as a pre-requisite of this. 

Nonetheless, health insurance for college students is still required by most education providers. In this case, it is generally managed through your college/university. If so, you will be automatically enrolled in a student insurance plan. However, you can sometimes opt out if you can prove you have equivalent coverage elsewhere. 

### The Schengen Area

The situation in the European Union is more complex. This is because there are essentially two layers of rules for countries within the Schengen Area: 

* **Schengen:** A minimum set of visa/insurance requirements applied across the whole region.   
* **Individual countries:** EU countries also have their own visa/insurance requirements. By design, these must supersede the Schengen minimum. 

The baseline Schengen rules are set out in [Article 15 of the EU Visa Code](https://eur-lex.europa.eu/eli/reg/2009/810/oj/eng) (Regulation (EC) No 810/2009). This includes the following requirements:

* **Coverage**: Must have travel medical insurance covering emergencies and repatriation  
* **Scope:** All policies must include coverage for all Schengen countries  
* **Financial coverage:** At least €30,000

In reality, most countries will have stricter requirements than this. In Spain, for instance, you need to have private healthcare for your visa to be accepted. It must also be valid for the entire length of your course. This can mean students have to buy insurance upfront, before the visa is accepted. 

France has a similar but slightly different situation. Here, you must also have valid insurance before you enter the country. However, it usually only needs to be valid for a few months. This is because you are eligible for coverage under the country’s social security system after a short period.

However, the state system only covers a portion of the costs. Therefore, you’ll generally require additional top-up (or ‘mutuelle’) health insurance. 

### United Kingdom

In the United Kingdom, visa applicants do not need to have international health insurance or private health insurance of any kind. This is because private healthcare in the UK is comparatively limited.  

Instead, you have to pay an upfront surcharge, which effectively ‘buys’ access to the country’s public healthcare system. This is known as the Immigration Health Surcharge, [which costs £1,035 a year](https://www.gov.uk/healthcare-immigration-application/how-much-pay#:~:text=%C2%A3776%20per%20year%20for%20students%2C%20their%20dependants%2C,for%20example%20%C2%A33%2C105%20for%20a%203%2Dyear%20visa.) in 2026. 

The charge must be paid upfront for the entire length of the visa. If you’re planning to study a three-year degree, therefore, you’ll have to pay £3,105 for three years’ access.

## How do I choose the right international student health insurance?

> *“The gaps that catch people off guard are almost always the same.*

> *First, transition coverage. You’ve landed in your new country, but you’re not in the local system yet. In Spain, you need your NIE (Número de Identificación de Extranjero, your foreign ID number). In Germany, public insurance can take weeks to activate after starting a new job. People assume they’re covered from day one. They’re not.*
>
> *Second, pre-existing conditions when switching plans. You had coverage at home and you get a new plan abroad. But you then discover your chronic condition has a waiting period of 12–24 months before it’s covered again. A short gap between policies can reset the clock entirely.*
>
> *Third, repatriation and emergency evacuation. Most people don’t think about this until something goes wrong. By then it’s too late to fix it.”*

When you choose your international student health insurance policy, it’s important to research the options on the table. The last thing you want is to take out an insurance policy that’s not valid for your visa, and then get rejected anyway. At the same time, you probably don’t want to pay for insurance that you don’t need. 

So how do you work out what the right level of coverage is for you? Here are a few important tips: 

### Public healthcare

In several European countries, you will be eligible for public healthcare after you’ve spent some time in the country. However, this is not valid from day one. Check the requirements in your country of study. This will inform what type of insurance you need and how long it needs to last for. 

### Length of coverage

If you’re likely to be covered under a public healthcare system, you may only need short-term ‘transition’ coverage. Otherwise, some countries (like Spain) will require insurance to be valid for the entire length of your visa. Understanding what you need is crucial to making the right choice. 

### Repatriation

Many standard plans do not include repatriation. If you’re studying in the Schengen Area, these will not be valid for your visa. Elsewhere, it’s sensible to make sure this is included even if it’s not a requirement. That’s because emergency repatriation can be very expensive. The last thing you want is to be landed with a multi-thousand-dollar bill after a medical emergency. 

### Pre-existing conditions

Medical insurance packages often do not cover pre-existing conditions. If you have particular ongoing health conditions, it’s important to be aware of the implications here. In some cases, you may have to purchase more expensive insurance packages to ensure you’re covered. 

### Additional coverage

You may also wish to be covered for additional procedures like dentists, orthodontics, or hearing support. In these situations, you may need a more expensive international student health insurance plan. It’s important to thoroughly check the coverage implications of whatever plan you choose.

## Why is standard expat health insurance often not enough for seniors?

Many retirees assume any international policy will do. In practice, a few things make this harder after 60 or 65.

### Age limits

Some international insurers set maximum entry ages, often 60, 65, or 70. If you’re past that threshold when you apply, you may be turned away. Always check the entry age before comparing providers.

### Pre-existing conditions

The older we are, the more medical history we carry. Many standard expat policies exclude pre-existing conditions outright or apply waiting periods. Are you a retiree managing a condition like diabetes, heart disease, or hypertension? This can leave a serious gap.

### Rising premiums with age

Standard expat policies are often tied to a specific country or region. Will your healthcare needs cross borders? Or will you want specialist treatment elsewhere, such as your country of origin? If so, that limitation can leave real gaps.

## Can you use Medicare overseas?

It’s the most important question for US retirees. The answer is mostly no.

Original Medicare (Parts A and B) doesn’t cover healthcare outside the United States. We confirmed via [Medicare.gov](https://www.medicare.gov/). There are only very narrow exceptions. One example: a foreign hospital closer than any US hospital during an emergency on US soil.

Living in Spain, Portugal, Mexico, or anywhere else outside the US? Best to plan as if Medicare provides no coverage at all.

Medicare Advantage plans (Part C) also typically don’t cover care abroad. Some include limited emergency travel coverage, but terms vary widely.

### What about Medicare Part B?

Medicare Part B [charges $202.90/month in 2026](https://www.cms.gov/newsroom/fact-sheets/2026-medicare-parts-b-premiums-deductibles). It can impose a permanent late-enrollment penalty. If you plan to return to the US eventually? It may be worth maintaining Part B. Check with a Medicare advisor before you decide.

We've established that Medicare provides little to no coverage abroad. So, most retirees use international private health insurance. It's their direct replacement while living overseas.

## What should seniors prioritize in a plan?

Pay close attention to these factors:

* **Coverage limits:** Aim for a high annual or lifetime maximum. Especially if you plan to return to the US for treatment.
* **Pre-existing condition terms:** Are pre-existing conditions excluded? Or covered after a waiting period? Or covered from day one? Ask this directly. The answer differs considerably between providers.
* **Emergency evacuation and repatriation (transport home if seriously ill):** Will you ever need medical evacuation from abroad? It can cost $25,000 – $250,000+. That’s according to [the CDC’s Yellow Book 2024](https://www.cdc.gov/yellowbook/2024/travel-by-air-land-sea/travel-health-insurance). Make sure it’s included in your policy, not just an add-on.
* **Prescription coverage:** Are prescriptions covered? Does coverage extend to medications purchased abroad? Be sure to check.
* **Lifetime renewability:** Insist on guaranteed renewability. You don’t want to be dropped mid-retirement because you’ve made a few claims.

### International private health insurance

International private health insurance provides healthcare for expats living outside their home country. It’s sometimes called global health insurance.

They typically cover hospital stays, outpatient appointments, specialist visits, and diagnostics. Prescriptions are often included too. 

### Local insurance in your destination country

Buying from a local provider can be cheaper. It also integrates well with the local healthcare system. But there are trade-offs. Coverage is limited to one country. Documentation is often in the local language. Eligibility rules vary. Worth exploring if you plan to stay permanently in one place.

### Travel insurance: not enough long-term

Travel insurance covers emergency treatment only. So, no routine care, no chronic condition management, no prescriptions. If you’re living abroad, it’s not a substitute for proper health coverage. For shorter trips, travel insurance is worth a look.

The right plan makes it manageable.

### What drives your cost

* **Age:** The single biggest variable. Expect premiums to increase meaningfully at each renewal as you age.
* **Coverage level:** A basic plan covering emergency and hospital care costs less than a full plan. Add outpatient visits, mental health support, and prescriptions and the price rises significantly.
* **Destination and area of coverage:** Plans including the US cost substantially more. As the Feather pricing above shows, adding US coverage can multiply your premium by three or more. Exclude the US from your coverage area if you don’t plan to seek treatment there.
* **Pre-existing conditions:** Your medical history may push premiums higher. It depends on how a provider assesses your health (called underwriting).

## A practical example: choosing expat health insurance with a pre-existing condition

### The situation

Maria is a hypothetical 67-year-old retiring to Lisbon, Portugal. She has well-managed hypertension and takes daily medication. Maria doesn’t plan to return to the US for treatment. She needs a plan that will actually cover her condition.

### The options

She looks at three options. A **basic international plan** covers inpatient and emergency care only. But it typically excludes outpatient care and prescriptions. Those are the two things she needs most.

A **comprehensive international plan** costs more. But it covers specialist visits and ongoing prescriptions. It also gives her English-language support from day one.

A **local Portuguese private plan** is even cheaper. But it only covers Portugal and paperwork is entirely in Portuguese. For Maria, the comprehensive plan is the right fit. The basic plan looks affordable but leaves her most important needs uncovered.

### The question that matters

With hypertension, Maria knows her condition will be scrutinized. Many plans will exclude it outright or impose a 3 to 12 month waiting period. So, before comparing prices, she asks every provider one question. “Will you cover ongoing hypertension medication and related specialist visits from day one?” Most can’t. The ones that can become her shortlist.

## How pre-existing conditions are handled

This is where most retirees get caught out. Here’s why:

### Medical underwriting

Most providers ask you to complete a health questionnaire when you apply. This is called medical underwriting. The insurer uses your answers to decide whether to cover you, at what price, and under what exclusions.

### Waiting periods

Some providers cover pre-existing conditions after a waiting period. That’s typically 3 to 12 months. During that time, conditions you had before taking out the policy won’t be covered.

### Guaranteed-issue plans

A small number of providers offer guaranteed-issue plans. These accept all applicants without a health questionnaire. They cover pre-existing conditions from the start. Such plans tend to be more expensive and relatively rare. Most plans are underwritten.

We asked Julian Hennig, Head of Insurance at Feather, for his thoughts. “For retirees over 65, the biggest mistake is focusing on price. Coverage stability is even more important. A cheap plan that excludes your most important conditions isn’t really insurance. It’s a false sense of security.”

## Which international health insurance plan is right for you?

Here’s what to do depending on your situation:

* **You’re 65+ with Medicare:** Get a comprehensive international private health insurance plan before you leave. Medicare won’t cover you.
* **You’ve got pre-existing conditions:** Don’t buy on price. A cheap plan that excludes your conditions offers no real protection. Look for guaranteed-issue plans or those with waiting periods under six months. Ask every provider directly: will my specific conditions be covered, and from when?
* **You’re retiring to a country with strong public healthcare (e.g., Portugal, Spain, France):** A lower-cost private policy covering the gaps in the local public system can work well. Avoid this route if you have complex pre-existing conditions. Or if you want guaranteed English-language support.
* **You plan to travel frequently or [split time between countries](https://feather-insurance.com/en-ww/blog/international-health-insurance-digital-nomad):** A worldwide policy is the best choice here. Avoid regional plans. They look cheaper but leave you uninsured the moment you cross into an uncovered country.

## How do you compare providers?

* **Deductibles and limits:** A lower deductible means less out-of-pocket cost when you claim. But it means higher premiums. Annual, lifetime, and per-condition limits vary too. Read the policy schedule carefully.
* **Direct billing:** Does the insurer pay hospitals directly? If not, you pay upfront and claim back. This matters most in a medical emergency.
* **Exclusions:** Watch for pre-existing condition exclusions. Also look out for country-specific restrictions. Remember conditions that arose before the policy start date.
* **Claims process:** Can you file online? How quickly do they pay? Check independent reviews before deciding.
