If you're self-employed, a digital nomad, or a retiree, voluntary SSN enrollment only becomes possible after your permesso is issued, takes 6–14 weeks to complete, and costs a minimum of €2,000/year.

Yes. You can join the Servizio Sanitario Nazionale (SSN) once you’re employed in Italy, have a residence permit, or register your residence. Students, retirees, and financially independent residents can also enroll by paying a voluntary contribution.

The minimum is €2,000 per year. The actual amount is income-based: 7.5% of income up to €20,658, plus 4% on anything above that threshold.
