Finding the [right health insurance in Panama](https://feather-insurance.com/en-pa/international-health-insurance-panama), though, is one step that can’t wait. Go without it and you could be on the hook for very large bills. Emergency surgery alone can reach USD 5,000 to 25,000. Medical evacuation can cost USD 20,000 to 50,000+.

## **Healthcare in Panama: how the system works**

Your options for healthcare in Panama depend on employment status and location. And there isn't a single system for health insurance in Panama for expats. There’s a public social security fund and a private sector. The private sector varies significantly by location.

### **Does Panama have public healthcare?**

Yes, but not to most expats. Panama’s public health system is the [CSS](https://www.css.gob.pa) (Caja de Seguro Social). It covers formally employed workers and their dependents only.

It covers the basics for routine care in Panama City. However, specialist waits stretch to weeks or months, and everything is in Spanish. For national hospital network data, see the [Ministerio de Salud (MINSA)](https://minsa.gob.pa).

### **What are private hospitals in Panama like?**

Panama City has excellent private hospitals. Hospital Punta Pacifica is a Johns Hopkins Medicine International affiliate. 

Then there’s Hospital Nacional and Centro Medico Paitilla. They're also well-equipped, with English-speaking staff and short wait times. Direct billing is available at major private facilities with most international insurance plans.

### **What about healthcare outside Panama City?**

Outside Panama City, healthcare infrastructure thins out considerably. Reaching a major hospital for serious care usually means a trip to the capital or a flight abroad.

Let's look at Boquete, for example, one of the most popular expat destinations in the country. It's 40 minutes from Hospital Chiriqui in David, which handles routine care well. Bocas del Toro and the coast have much less.

## **Your options for health insurance in Panama**

### **CSS (public system, employed workers only)**

If you’re working for a Panamanian employer you’ll be enrolled in CSS automatically. You contribute 9.75% of salary and get access to the CSS hospital network.

CSS is employment-based, not residency-based. That matters: Pensionado visa holders who aren’t employed in Panama have no access to it. Retirees, remote workers, and digital nomads won’t qualify.

### **Private health insurance in Panama**

Private health insurance in Panama is available through local insurers. These include PALIG (Pan American Life), ASSA, and Mapfre Panama. All three are built around Panama City’s private hospital network. They're regulated by the Superintendencia de Seguros y Reaseguros de Panamá. Annual premiums run from around USD 1,500 to USD 6,000 depending on age and plan level. You get faster specialist access than CSS. Importantly, English is standard at major facilities.

But there are real limitations. Coverage is Panama only, with no portability. Most Panamanian private insurers stop accepting new applications somewhere between ages 65 and 70\. PALIG is the most widely used among expats, but premiums rise sharply after 50, and new enrollment is typically unavailable beyond 65\. Pre-existing conditions are usually excluded outright or subject to waiting periods.

For healthy expats in their 40s or 50s, it offers solid value for in-country needs. But beyond the 65–70 enrollment cutoff, expat health insurance in Panama via a local insurer is often simply not an option. 

### **International health insurance in Panama**

International health insurance in Panama covers you worldwide, the US included. Panama City is a short direct flight from Miami. If you travel regularly between Panama and the US, repatriation coverage is worth having. Feather’s plans accept new enrollees up to age 75\. That’s much older than local Panamanian insurers offer. Plans start from EUR 85 per month. You get English-language support and direct billing at major Panama City hospitals. Pre-existing conditions are assessed individually at application, so you get a clear answer upfront, rather than an automatic exclusion. There are no network restrictions and your cover also goes with you if you move. 

There are some limits worth knowing. Pre-existing conditions must be disclosed at application, US cover is for emergencies and accidents only, capped at 60 days per year. Home-country stays are capped at 180 days.

## **Panama health insurance for retirees: the Pensionado visa explained**

### **What does the Pensionado visa include?**

For Panama health insurance for retirees, the Pensionado visa is where to start. It’s Panama’s official retirement visa, and one of the most generous in the world, established under Law 6 of 1987\. A guaranteed pension income of USD 1,000 per month qualifies you for permanent residency. 

The benefits include 15% off hospital bills, 20% off prescriptions, 25% off airline tickets, and more, enforced by [ACODECO](https://www.acodeco.gob.pa). 

Health insurance isn't a general legal requirement in Panama, according to [migracion.gob.pa](http://migracion.gob.pa). However, some types of visa do require proof of coverage, such as the Friendly Nations Visa. ### **Does the Pensionado visa include health insurance?**

No, and that’s what catches people off guard. CSS access is not included. Pensionado holders who aren’t employed in Panama cannot use the public health system. The hospital discount and CSS membership are entirely separate things.

### **How does the 15% hospital discount work?**

It applies to your gross bill, not to insurance copays or claims. A USD 12,000 procedure at a private hospital becomes USD 10,200 with the discount applied. Without insurance, you still owe USD 10,200 out of pocket. The discount reduces uninsured costs; it doesn’t replace coverage.

### **What if you’re over 65?**

Most Panamanian private insurers close new enrollment at 65 to 70\. A 67-year-old applicant is likely to be declined by PALIG, ASSA, and most local providers. International health insurance, with higher age limits, is often the only realistic option.

**Pensionado retiree, under 65, good health:** A Panamanian private plan offers good value for in-country care. PALIG is the most popular with expats. Add international cover if you travel to the US regularly.

**Pensionado retiree, over 65, or pre-existing conditions:** International insurance is likely the only option. Local insurers cap enrollment at 65 to 70\. 

**Employed expat:** CSS covers you from day one. Most people in this situation top it up with private or international cover. That depends on travel frequency and coverage needs.
